'Why can't all Americans have the same access to healthcare to those enjoyed by members of Congress?' is a popular question on the ObamaCare debate.

At the time of writing I am following the debate over universal healthcare for all Americans. If the US$1 trillion Bill passes, it will help insure 32 million Americans that do not have access to healthcare.

This is another controversial issue in the tradition of Democrats and Republicans. This is a good case study of one of the enduring issues of an advanced capitalist state.

I know friends who do not have health insurance and who question the human rights dimension of it - right to life, liberty, and the pursuit of happiness, endowed by the Creator who insist that 'all men are created equal' and cautioned by the Enlightenment thinker Jean Jacques Rousseau that “… everything is good in the hands of the Author of Things and everything degenerates in the hands of Man”.

From the point of view of the Democrats, the health insurance system is broken and in deep crisis. Millions cannot get proper medical health. The insurance system is predatory. Expanding health coverage and lowering prescription drug prices, and giving rebates to insurance companies are the main features of this proposal.

From the Republicans' point of view, Americans need health reform but not to the point of bankrupting the country. A Bill which covers the cost of abortion is the controversial part of it; that Americans must not pay for those who do not have the respect for life. The Bill is said to be a wrong approach that will make winners and losers in the system. Essentially it will open up other complications in virtually all aspects of the system.

Whatever the outcome of it, Americans will still be divided ideologically on this issue. The argument is emblematic of the American political philosophy: what is the role of the government vis-a-viz the social contract between the 'ruler and the ruled'?

It is a classic Jeffersonian-Franklinian debate. America has evolved into a country envisioned by Benjamin Franklin - America which is more of the big business and less on the man on the street.

Americans are taught to not trust governments; its history is that of a revolt against British colonialism famed by the slogan 'taxation without representation' and therefore 'give me liberty … or give me death', as the revolutionary leader Patrick Henry said.

Americans went to war and bankrupted the nation. It was a Republican war. That drained trillions of dollars, perhaps contribution to the near-collapse of the American Empire, as many a Complexity theorist would propose. The Butterfly that flapped its wings in Baghdad, near Saddam Hussein's mansion has contributed to the turmoil in the Obama Office.

Rights of all Malaysians

But what is the situation in Malaysia, as we have evolved as a modern state enculturalised by happenings in other advanced countries such as the US?

How do we care for the sick? Essentially is there also a class system in our healthcare system? Do the poor get the same treatment as the wealthy members of parliament or those in the list of billionaires?

With the proliferation of private hospitals, are we creating the foundations of a class system that will inherit the problems the Americans are trying to resolve?

Or is this merely a natural progression of an economic system that is also predatory in culture - that the rich will be richer and the poor growing in numbers?

With the urge for Malaysian private hospitals to venture into 'medical tourism', will our good doctors abandon their Hippocratic Oath in favour of professional hypocrisy?

Marx would say that we are defined by the economic condition we are in - we are homo economicus. I suppose how we live and how we die and how we are taken care of in-between this period of 'borrowed time', depends on how the state defines what human rights mean vis-a-viz our ability to pay for healthcare and how we lived our lives as a economic beings.

We must consider that each human being is a cog in the wheel of Capital. Those who own the machines of production oftentimes influence policies through political-economic arrangements.

A wealthy country such as Malaysia that prides itself on tall buildings and a growing number of billionaires ought to start reflecting on the need to ensure that each citizen will have affordable healthcare.

One wonders what the limit of wealth creation is and where the moral dimension is in capitalism, when the rich control the lives of the poor.

By shaping ideology and creating installations to change the social relations of production - and by doing this through the control of mind, media, machinery, and materials - we expect that wealth is to be shared. Socialism for the rich must be replaced with capitalism for the poor.

It's 11pm here in New York. The historic Bill has just been passed, 219 to 212 in the House. God bless the life of 32 million more Americans. Let Malaysia learn from this victory!

I think it is the "great" nation of USA that should learn from others in term of health care. It can start with its neighbor Cuba. American doctors and insurance leeches are driven by greed not the patient's needs.

Do you know that even you dont have insurance you can get medical treatment in MALAYSIA??

Apa yang bagus dengan AMERIKA?? Nothing because everything needs MONEY... No SUBSIDY.. If they do it is for RICH PEOPLE..here almost everything been subsidies by MALAYSIAN GOVERNMENT..

Please Doc..please be a DOCTOR not a POLITICIAN.. for me,myself as a RAKYAT i know what my MALAYSIAN GOVERNMENT give me..please dont lie..

If i dont have money i will go to GOVERNEMENT HOSPITAL..if i do have money (or insurance) i will get it for PRIVATE HOSPITAL..

By saying so not because Government Hospital lack of qualitie or services BUT at Government Hospital because of RM1 registration fee and free medical treatment..so many people come to get their treatment there,i go to the PRIVATE HOSPITAL because only t avoid that queue..

So dont compare MALAYSIA to AMERICAN becaus THEY should learn from MALAYSIA the meaning of MEDICAL ACCESSABILITY..

So DOc if you have money and want to go to the PRIVATE HOSPITAL please do so.. nothing will prevent you to do so..

So do we,those who cant afford to buy insurance because RM300 medical card is consider expensive to us,so we go to GOVERNMENT HOSPITAL that charge only RM1..

Thats the flaw of the american Democracy whereby lobby groups has power over elected representatives regarding the legislative foreign policies. Thoes laws andpolicies are made in teh interest of these lobbies. You should watch blade runner instead of matrix and see how corporations are taking over goverments and the world. If Dr.Azly lobbied hard enough and gave up enough political contributions, I'm sure matrix trilogy will be acknowledged as a fact in american history.Is that the sound of the acme dynamite goin off doc? kaboom!

I'm asking this because a friend of my friend is coming back from the land of plenty after living there for over 20 years. Reason? Medical and living costs,Also being unmarried so no one to take care of him. In a society that is supposedly just, advanced,and operates by the rule of law principle where is the compassion for the weak?

25 million homeless in the US alone. Queuing with food coupons for food everyday.

these people are still not covered by the new health bill Obama just signed.

During Azly's era the quality of doctors being churned out by the day is a big question mark. I dread to visit the government hospital being attended by those young doctor graduates of the Russian, Indonesian, Indian universities.No choice but to take up a medical insurance.But what choice do the unaffordables has?

Dr Azly, sorry to be slightly off topic. Upon all the comments posted here, have you not convinced that malaysian medical care, is better than american's? I read your profile, you are specialize in education, english, and so called 'brain science'.

So is there any chance the readers could at least see the day where you admit your mistakes to your reader?

I have not been to USA, i've seen SICKO documentary by Michael Moore. One of the story Moore depicts is about a guy who must choose only 1 finger to save out of 2 fingers which got cut in an accident because of his insurance policy. Also about a sick lady got thrown to the streets because she got no insurance.

Our government hospital service is bad not due to our health policy, but bad management and lack of manpower. If we can fix this somehow, it will be as good as private hospital.

I will share you my experience in KPJ healthcare in Shah Alam section 20. I have a motorcycle accident, twist my wrist. KPJ cost me 170 for x-ray, bandages, and medicine. But this is just in the emergency dept, doctor recommends me to meet their specialist(to milk me for more money). I have company insurance, but for emergency i need to fork out my money first and claim.

I refused to meet their specialist, i went to Hospital Serdang, pay 1 ringgit, wait for about 5-6 hours from morning (they focus on the serious case first); get to meet specialist another new set of bandages.

Yeah it sucks to wait for 6 hours, but what to do? At least i got mc and proper medical consultation for pretty much nothing. What can you buy for 1 ringgit nowadays? Even teh tarik cost more.

So Dr Azly, please educate me!

But you have a long way to go in terms of mastering the way to present good arguments. You obviously do not understand the complexity of my articles in this blog. You obviously do not know why I am writing -- all these over 300 articles on social change.
These are troubling articles to many, because we must deal with troubling issues of our times.

wealth is to be shared... in the uk,they have tax credit collected and distributed to the one in needs..they are actually doing the zakat stuff although its a non muslim country.
what happen to tax we paid? and also zakat? arent they suppose to help the poor?and upgrading the hospitals? where does all the money go?

we need someone to stand up and help do something abt this. this is urgent matter

I am not rich and we had to send our grandmother to University Hospital at UM, a government hospital for cataract surgery.

We had to to wait for 5 hours for consultation

We got the consultation just as well from specialist.But have to wait 2 weeks for the surgery.

2 weeks later, went to UH at 7am, waited for 6 hours for my grandma's turn.Eventually got her turn and did the surgery.We didn't py first, UH did the surgery first then we pay.

Enlighten mr DR Azly, this was 2 weeks ago,my grandma got her surgery and it cost only RM800 including the medicine for 2 months and even prescription glasses.Do I need citations and reference to proof to you that our health care system is in limbo.FTW, 4000 medical students sponsored by the government are being trained abroad to address the doctor shortages, we are heading in the right direction far longer than the US.

Despite stressing argument it is you who not have the argument Dr Azly..what proof do you have that our health care is soooo inferior compared to the US.Privatisation? The government despite all its fault at least stopped the IJN from being pawned to the private sector, guaranteeing that Malaysian could afford heart treatment at low cost.The UH is being modernised and expanded to add more beds and government hospitals around the country are also being upgarded..not to mention the various Klinik Kesihatan Kerajaan including 2 around my area.

We may not have the best health care but it is certainly not as bad as the US.I pity US doctors for letting their patients die for not having a plastic card.

And BTW, I just paid RM500/year premium for my medical insurance this year..access to hospital and all forms of surgery as long as I am hospitalized.And 30+ million US citizens can't afford medical insurance..why?

Dr Azly..you have lived in the US far too long....do come back and see for yourself before 'arguing'.

What next? Government making Malaysians fatter with palm oil?????

Please visit government hospitals once in a while...they may not have the freshest coat of paint but our system works as well as any Western hospitals. The UH at UM is undergoing expansion and refurbishment..yes tax payers money are being spent on our hospitals more than we spend on defense.
